  • Requisitions could be obtained from the office for mammograms, bone mineral densities, FOBT, ECG’s, and other screening tests. NO appointment is necessary to obtain requisitions.
  • Patients with chronic illnesses, for example, diabetes, chronic kidney disease, hypertension and ischemic heart disease, should be seen at the office every 3-6 months.
  • Follow up appointments for these chronic conditions, should be made while you’re at the office for your current appointment.

  • The annual flu vaccine will be seasonally available. Our website will be updated with flu shot clinic times when available.
  • Please contact your pharmacy if you need a repeat prescription.
  • Special investigation results will be reviewed on a daily basis.  Patients will be notified in the event of abnormal results.  Special investigation results are available at the office if patients wish to obtain a copy.
